If you would like to swim at Hidden Valley...Here is the waiver (page 3-4 for students, page 1-2 for adults who will be staying for practice) and the COVID-19 acknowledgement form. Coach Donna will need a copy of the waiver for each student who will be participating in practice throughout the month. She also needs a copy of the adult waiver for any parent who will be staying for practice.
