We are hiring a Full Time RN Hospice Case Manager Responsibilities: Travel is required and travel expenses will be provided Plans and directs services that are personally tailored to meet the unique needs of our patients and their families by assessing their physical, emotional, psychological and spiritual well-being through on-going, comprehensive nursing assessments.... Provides direct, skilled nursing services in accordance with the Plan of Care and evaluates the efficacy of planned care interventions, treatments and medications. Documents physical assessments, skilled care/interventions, verbal and written orders, progress towards problem resolution and updates care plans at the point-of-care and in compliance with professional scope, Agency policies and procedures and state and federal regulations. Collaborates with the Hospice Interdisciplinary Team (IDG), patient and caregiver to ensure the highest quality-of-care and achieve optimal patient outcomes. Identifies need for and coordinates appropriate patient and family centered health teaching. Is able to work effectively as a part of a team and independently with minimal supervision. Possesses strong written and verbal communication skills.
