Countrywide Home Care is looking for a Bilingual (Spanish) Case Coordinator /Scheduler with Support Coordination experience in home care. An excellent candidate is organized, detail oriented, self-motivated, professional and has great communication skills on the phone and in person. Must be comfortable dealing with people, performing administrative tasks and multi-tasking with speed and accuracy. Hours for this position are Monday through Friday from 9a-5p, with additional on...-call responsibility. Responsibilities and Duties Communicate with MCO/support coordinators/consumers/caregivers and document it all in the internal system and HHAeXchange; Review and resolve consumers' complaints, keep the log up-to-date; Place the Welcome calls to the new consumers; Schedule the 1st home visits; Ensure authorization reflects stated hours for consumers once authorization is obtained so care can be provided correctly; Staff the cases and set up meet & greet if needed; Confirm with Support Coordinators increase and decrease in consumers' hours; Inform both consumers and caregivers about changes; Participate in on-call rotation. Qualifications High Communication Skills; Strong computer skills including Word and Excel; Ability to multi-task; Knowledge of medical terminology; Home care experience required; Support Coordination experience required. Bilingual (Spanish, English).
