Attention winter sports athletes and parents: Starting this year and moving forward we will be handling physical paperwork digitally. We are not accepting paper copies of any form at the high school. Please follow the instructions below on how to complete and turn in your sports physical for the upcoming season. In order to participate in open gym or preseason workouts you must have the COVID-19 waiver signed online. Pages 1-6 of the sports physical, the COVID-19 waiver and drug & alcohol form must all be completed by the first practice or you will not be allowed to participate.

We realize the unique challenges faced by families and school districts during this time and have teamed up with your school to keep our promise of delivering free, convenient physicals for all student-athletes. Typically, physicals are provided at schools but with the... influence of COVID-19, we will instead be offering free physicals at many of our convenient locations. These physicals will be done on an individual basis and appointments will be scheduled ahead of time. Appointments will be scheduled starting on October 19th, and will run through the beginning of winter sports. How does this benefit you, the athlete? Our updated system is: Convenient. Service Electric Cablevision (SECV) is going to again offer the eLearning promotion which entails the following: 90 days free Internet service to students and... teachers who do not already subscribe to SECV Internet. This offer includes the 40M x 4M tier of Internet service, along with a free cable modem rental. Self-installation required with the assistance of a technician outside the residence to ensure the safety of our employees. All new customers must provide written proof of their child’s current enrollment at a local school in SECV’s service area. All teachers must reside in SECV’s service area and provide evidence of current employment status. Other restrictions may apply. The PIAA has pushed the start date for fall sports back 2 weeks. This means we will continue fall sport voluntary workouts until the new official start date on August 24th. Athletes need their sports physicals turned in before that date to participate. Only the COVID-19 waiver and daily screening questions are required for voluntary workouts.

The outbreak of Covid-19 has caused unprecedented change in all aspects of life, and obviously athletics is no different. It is with that in mind that LVHN’s Athletic Training Department would like to offer telemedicine services for your student athletes. How it works: First, all high school and middle school student athletes and their parents will have to sign an informed consent form to allow their child to participate in telemedicine services. This can be done digitally th...rough a link when booking your appointment. Setmore.com is a web based scheduling service that our department is utilizing to schedule telemedicine visits. Setmore allows for complete customization of schedules (days/hours that the staff is available) as well as what types of services are offered such as injury evaluation, supervised rehabilitation, etc. The video portion is being hosted by Teleport video. Teleport is an integrated video conferencing app that utilizes secure end to end encryption ensuring the privacy and confidentiality of the visit. For the first visit, we ask that the parent/guardian of the student athlete be present. All visits conducted in this manner will continue to be documented just as any other in person visit with the athletic trainer would be. This telemedicine service will continue to be available even after the current health crisis passes, enabling your student athletes to continue to get the prompt, cutting edge service that you are accustomed to. Highlight the event you wish to observe in red or just come to the office after school and you will be placed at whichever event will deliver the best learning experience that day. On next Wednesday, March 4th, we will have a 15-20 minute group Orientation to kick off the spring sports season. All members interested in shadowing spring sports should attend. We will take a look at the facilities here at the high school for anyone who hasn't seen them and go over all pertenant information before we start the shadowing program. If you cannot make it that day we can arrange to have a meeting and go over any information you missed.
