Substance Abuse Counselor needed for the Kensington Hospital Outpatient Treatment Program on a full-time basis. Hours are: 6:30 AM - 2:00 PM, Monday- Friday. With being in our immediate location we provide expertise in the field of Behavioral Services making Kensington Hospital a premier care facility. Duties include, but not limited to: Maint...aining a caseload of 35 clients, meet with newly assigned clients within twenty-four (24) hours of admission, responsible to complete the Psychosocial History and Evaluation on clients, develop an individualized treatment plan, provide individual/group counseling services to clients, complete necessary forms required for record retention, including, but not limited to, counseling progress notes, ASAMs, treatment plans, Bio Psychosocial/CBE, activity oriented progress notes, quarterly and semiannual and annual reports, discharges summaries and aftercare plans per regulatory/funder requirements, consistently completes assigned/required tasks.
