Refuse vs Decline I just saw a nice little video about this and it reminded me to elaborate on the difference between the two! In healthcare, the use of the ...word ‘refuse’ instead of ‘decline’ really boils my pi**. While the two have roughly the same broad meaning (saying no to an offer/invitation/even recommendation), the associations in our heads regarding the two words can be very different, and that hugely influences how we perceive the person who is refusing/declining something, with a potential impact how we think and feel about them, and how we may offer care to them and treat them in the future. Even if it’s not intentional, and more subconscious. Refuse has associations of unreasonableness, childishness, pigheadedness, being obstinate, and possibly rude. Decline is polite, formal, quite reasonable and has absolutely no undertones of rudeness or unreasonableness. Basically, ‘refuse’ in many peoples heads is similar to: Absolutely f***ing not, are you kidding? NO!!!’ maybe with a sullen expression, and potentially in a raised voice (btw, having to do this as a pregnant person is also absolutely fine....if we are not respecting autonomy and choice, we have to be prepared to be put in our place!). Whereas ‘decline’ is a simple and considered: ‘No, thank you.’ possible delivered with a smile. I know this is hyperbole , but really, the two are very different in a healthcare context. So it would be wonderful if we could consider this when documenting or talking to colleagues. While the ‘refuse’ terminology might not have hugely negative connotations for YOU, it does for many people, some of whom may be reading your documentation, or receive your handover. And a person who is considered rude, stubborn and unreasonable DOES receive different care and communication from a person considered to be polite and reasonable. (Wherever that may mean.). There is also the power imbalance that gets highlighted here. As already mentioned, refusing to do/accept something assumes that it really OUGHT to be accepted, which assumes a: the HCP is right, and b: they have the power to decide what should happen. The balance of power is tipped towards the HCP . Being able to decline puts the power firmly into the decliners hands. As it should be . So yeah, there IS a difference, even though the basic meaning is very similar. It’s worth remembering that (illustration my me )

Plasticenta: First evidence of microplastics in human placenta Abstract Microplastics are particles smaller than five millimeters deriving from the degradation of plastic objects present in the environment. Microplastics can move from the environment to living organisms, including mammals. In this study, six human placentas, collected from consenting women with physiological pregnancies, were analyzed by Raman Microspectroscopy to evaluate the presence of microplastics. In to...tal, 12 microplastic fragments (ranging from 5 to 10 m in size), with spheric or irregular shape were found in 4 placentas (5 in the fetal side, 4 in the maternal side and 3 in the chorioamniotic membranes); all microplastics particles were characterized in terms of morphology and chemical composition. All of them were pigmented; three were identified as stained polypropylene a thermoplastic polymer, while for the other nine it was possible to identify only the pigments, which were all used for man-made coatings, paints, adhesives, plasters, finger paints, polymers and cosmetics and personal care products. Analysis of microplastics by Raman Microspectroscopy Results: From each placenta, three portions with a mean weight of 23.3 5.7 g were collected from the maternal side, the foetal side and the chorioamniotic membranes. All portions were opportunely processed for the subsequent analysis by Raman Microspectroscopy. In total, 12 MP fragments (named #1#12) were detected in the placentas of four women; more in detail, 5 MPs were found in the foetal side portions, 4 in the maternal side portions, and 3 in the chorioamniotic membranes. Microplastics #1#4, #6, #7, and #9#12, were ~10 m in size, while #5 and #8 ones were smaller (~5 m). All the analyzed MPs were pigmented. The report includes a description of each MP In conclusion, this study sheds new light on the level of human exposure to MPs and microparticles in general. Due to the crucial role of placenta in supporting the foetus development and in acting as an interface between the latter and the external environment, the presence of exogenous and potentially harmful (plastic) particles is a matter of great concern. Possible consequences on pregnancy outcomes and foetus are the transgenerational effects of plasticizer on metabolism and reproduction (Lee, 2018). Further studies need to be performed to assess if the presence of MPs in human placenta may trigger immune responses or may lead to the release of toxic contaminants, resulting harmful for pregnancy.

Quickie Quiz Question. How long does it take for a healthy, normal newborn to achieve an oxygen saturation of 90%? . . . .... Answer. 10 minutes! Can you believe, it might take up to 10 minutes for a newborn's system to achieve an oxygen saturation greater than 90%? (NRP 7th Ed p. 54). In utero, humans have an oxygen saturation of about 60% during gestation. Babies are designed to handle these lower 02 saturations. That number can drop to 10% while the baby is passing through the bones of the birth canal, during the squeeze of a contraction. After the contractions, the 02 saturation goes back up. This is normal. . When a baby is born, as they transition to being an air-breather, and start oxygenating through their lungs and not through the placenta, it takes a little time (~10 mins) for the 02 saturation to increase to about 85-95%. . Did you know that trans and non-binary people are more likely to select non-hospital or community-based options when giving birth? They are also more likely to ...request a caesarean birth, and are less likely to chestfeed. But today, we want to focus on the tendency for trans and non-binary folks to opt for community-based birthing practices. There are many reasons for this decision uninformed and biased providers in traditional spaces that are ill-equipped to administer care properly, past trauma that render hospitals outside of one's safe space, and the inability to escape sexist and/or transphobic violence. Preparing for pregnancy as a non-binary person involves extra steps to make sure your mental and physical health are top priority and supported throughout the entire pregnancy and birthing process. Research shows that it's helpful to have an affirmation of identity, affirmation of pregnancy, and a feeling of physical and mental safety. Most often, trans and non-binary people find this within their community, and pregnancy experts outside of the hospital world. If you're experiencing a #transgenderpregnancy or #nonbinarypregnancy, we encourage you to find and lean into your pregnancy community. Explore doulas, midwives, or the like that are trained in trans and non-binary care practices. Whether you choose to give birth in a hospital or not, having this community surrounding you throughout your pregnancy can help immensely. Baby steps! Pennsylvania Department of Health has released their updated "gender neutral crib card." They use "parent" up top, but "mother's" down below (I've edited this image to have it state "Birth Parent's") and it includes "chest" for chestfeeding parents! Once the DOH understands that not all parents are "mothers/fathers," maybe we can get better forms across the board from fertility & family planning to perinatal care to early postpartum care and finally to interaction...s with pediatrics until our kids age out. Myself and other providers of all kinds that I know will keep pushing and challenging our government and health systems to change their language, forms, and care to align with cultural competencies and affirming language. But it's patients, parents, and caregivers who have to speak up, to anyone who will listen, at every opportunity possible for these changes to be made a priority in our government and health systems. We have another educational video! This one has a twist--the baby was an undiagnosed complete breech, and the midwife only realized this right when the legs po...pped out! This was the midwife's first breech birth. She told us that she was freaking out internally, but you'd never guess it from watching the video. This baby also did something a bit different: after the legs were born, the baby stayed sacrum transverse and even started rotating slightly towards sacrum posterior (SP) with the second contraction. At this point, the midwife gently swept down both arms, which helped the baby rotate back to SA. The head came immediately after. We include some discussion at the end of the video whether sweeping the arms was indicated or not. Even with a team of breech experts, they did not all agree. What I love about this video: it can help us reflect on how we might respond in a similar situation--under pressure, with no advanced warning.
