How do you know which nerve root becomes affected in a disc herniation? Generally, there are two areas of compression: The descending lateral recess, and the fo...raminal exit. For example: -- The L4-5 lateral recess will affect the L5 root. (descending root) -- The L5S1 foramina will affect the L5 root. (exiting root) Thus, central disc herniation will affect the descending roots whereas foraminal stenosis will affect the same-level exiting roots. Lateral recess (central or para-central) hernias generally worsen with spinal flexion. However, stenosis at the foramina is generally caused by arthritis and would worsen with spinal extension. A broad based central foramina of the L4-5 level would often compress both descending L5 roots and cause bilateral L5 dermatome pain along with potential L5 myotome weakness (dorsiflexion). Moreover, it would produce a bilaterally positive Lasegue's test. Foraminal narrowing would not be able to affect both roots unless both foraminae are indeed narrowed. Radicular (dermatome-like) pain upon spinal extension is common for foraminal arthropathic stenosis. Generally, I'll do the myotome examination first, have the patient hyperextend the spine, and then redo the myotome tests. If weakness now appears, and it fits with the MRI (or report), it is likely to be arthritis rather than discogenic compression. -- Large disc herniations with radiculopathy, as long as there's no serious weakness, tend to heal nicely with conservative therapy. -- Small disc herniations with radiculopathy, with or without motor deficit, tends to heal poorly with conservative therapy due to congenital narrowing of the spinal canal (think about it: why would a small hernia cause root compression?) -- Foraminal arthritis with radicular affection can be treated short term by INCREASING spinal flexion in posture. However, this will speed up degenerative spinal pathology and generally it would be recommended to perform foraminotomy.

Here’s a book by Laban 1926 a year after Joe came to the USA. Gymnastics and Dance is the title. Lots of pictures of naked, semi naked, clothed dancers. Not o...ne picture of an exercise that looks like anything Joe pilates documented as his system. So the stories of Laban and Joe working together or influencing each other appear to be stories and not fact. Other wise why wouldn’t Mr Laban mention Joe in a book about gymnastics?? I don’t read German but I went through the book and did not see the word pilates in it Now I could have missed itbut don’t think so. Also historically we know that the dancers didn’t start to go to Joe until the 1930’s and later. So if one influenced the other they did not include it in their writings that I have seen. We do know Irmagard Bartenieff (not sure of the spelling) who was a student of Laban and did a lot to promote his work in the US was also a student of Joe’s but not until the 60’s but before she wrote her book(which also doesn’t mention Joe) but has exercises that resemble something along the lines of what Joe taught than what is seen in this book by Laban. So as of now the evidence suggests they most likely didn’t collaborate and or influence each others work The deeper we dig the clearer the history unfolds See more

Thank you Jocelyn Olivier! I have referred a lot of people to this exercise to see if it changes their ability to do breath holding out test for O2 retentio...n. The reports coming back say it's helpful. There have also been some collateral links on the post about Hidden Hyperventilation that point to the importance of Nitric Oxide in our regulatory systems. BTW...Some excellent links on the science behind all this from Rosalba Courtney who is hosting Breathe Hawaii 2021, Lois Laynee of the AZ Sleep Apnea Center, and Emily Splichal of EBFA were put on the discussion of HH. If you haven't tried this exercise yet, it is best done right before bed IMO. Breathing and Humming to Improve Vasodilation: Humming while breathing out is a gentle way to begin shifting your nervous system to rest and digest, where it needs to be in order to heal I use this simple technique every day with clients to teach them how to control their own Autonomic Nervous System ANS. I learned it from Lois Laynee of Restorative Breathing. I recommend studying with her. You could save lives. --Breathe in slowly and silently with lips closed, keeping your tongue on the roof of your mouth. Breathe out humming constantly. As you feel yourself running out of air, find your swallow. If your mouth is dry and there is nothing to swallow force the mechanism to work. Fake it. Repeat 4 times. (3 works so 4 is one extra.)-- When finished breathe quietly/silently and notice any subtle sensations throughout your body. Some folks feel a flush of warmth. Some folks can feel light tingling. Both of these are signs that the circulation is opening up. Some folks just feel more relaxed, so this is a very good thing to do right before dropping off to sleep. When your body makes the switch to the Parasympathetic Nervous System PNS, one of 2 branches of the ANS, your mouth will water as you reach the end of your humming exhale. That’s how you’ll know you’ve finally begun to re-regulate. Swallowing spontaneously ONLY happens when the body is in the Parasympathetic side of the ANS. Stress has Immediate Metabolic Effects: Have you ever noticed when you become aware of some really bad news that your mouth becomes instantly dry? We also know that digestive secretions are impaired when the body is stuck in a state of Hyper-Arousal. The implication of both these things is that when we are stressed we can't digest our food. The body doesn’t even want to swallow... The Science: A "panting shortness of breath" is associated with Sympathetic Hyper Arousal. Our flight/fight/fear system is being stimulated. Slow Diaphragmatic Breathing circles us back to our Parasympathetic side. These two sides of our Autonomic Nervous System ANS produce entirely different chemistries. One is geared to convert muscle into glycogen for immediate survival energy, the other into digestion, healing and cell repair. Humming while breathing out causes a proliferation of Nitric Oxide in your blood. Nitric Oxide is the body’s natural vasodilator. It is responsible for making the vascular walls more porous in order to have better Oxygen pick up from the lungs and to distribute more oxygen out of the blood to the tissues in our extremities. Use this technique with yourself, your family or with your clients to show them a quick and gentle way out of the stress system. For a live illustration of what we have described here....
