By the government’s regulation, we will conduct verbal interview asking you to sign in our check list when you come in;... 1. * Have you traveled outside of the state or country within the last 14 days. 2. * Have you experienced any COVID-19 symptoms (fever, cough, trouble breathing) in the past 14 days. 3. * Have you had any contact with anyone with a confirmed or suspected exposure to COVID-19 within the past 14 days. Even It may not be required by the Sate, we will conduct body temperature screening at the entry. Please arrive on time for your appointment.
