Job Title: CNA - Home Health Job Location: East Stroudsburg Office - Monroe County and surrounding areas Shift(s): Monday thru Friday 8:30 - 5:00p Traditional Home Health Care is committed to providing the residents of the community we serve with quality home health care, while providing a rewarding experience for our care givers.... Job Title: LPN - Home Health Job Location: Clarks Summit, PA Shift(s): Monday - Friday 8:30am to 5:00pm Job responsibilities include, but not limited to the following:... Prepares visit report, patient updates and summaries, and confers with other members of the health care team to provide optimal patient care. Requirements: Current license as a Licensed Practical Nurse in Pennsylvania Acute care and/or home health experience is preferred. Excellent assessment skills, documentation skills and ability to meet deadlines Must have a current drivers license Ability to travel to case assignments as assigned.
